A record of the obstacles I encountered while writing MCP servers, tool-calling loops, and RAG pipelines in production. Streaming failure in Anthropic SDK 0.39 → 0.42 migration, checkpointer race condition in LangGraph 0.2.x, 768 vs 1536 dim mismatch in Pinecone p2 index, empty response with tool_choice="required" in vLLM 0.6.3 — all with actual error messages and working fixes. Specific things that gave me the feeling of "I've seen this error before" while writing agents.
